Tools for
Patient Care Providers

HACPM offers providers several tools to assist with patient care. Please note that while these tools were developed and tested by experienced medical practitioners, they are intended as aids only and not as authoritative resources.

Drug conversion calculator
A calculator for converting drug dosages and intervals. Two versions of the calculator are available.
 Version 2.0 for Java 1.1 Version 2.0 requires Java 1.1 enabled browsers (for PC's using Netscape 4.03, 4.04, 4.5 or later, Internet Explorer 4.0 or later; for Macs using Internet Explorer 4.0 or later). It provides enhancements and added features including prn drugs, activity logs, and choice of conversion tables.
 Version 1.5 for Java 1.0 Version 1.5 can be used with all other Java enabled browsers. It provides updates and performance improvements to earlier versions.

 Dermatome map
An interactive tool for identifying dermatomes based on user selected body sites. It requires a browser supporting Java 1.1 such as Netscape 4.03 or higher or Internet Explorer 4.0 or higher.

Pain assessment tools
The pain assessment tools are provided to assist the clinician and patient in assessing pain. These tools are widely used in clinical practice. These tools are recommended in the AHCPR Clinical Practice Guideline for use as part of patient evaluation and assessment.
Brief Pain Inventory A short form for assessing pain produced by the Pain Research Group, Department of Neurology, University of Wisconsin-Madison.
Initial Pain Assessment Tool A pain assessment tool developed for use in initial patient evaluation (McCaffery and Beebe, 1989)
